form below. There is never any cost or
obligation.
All
information with an asterisk * is
mandatory. Not filling in all mandatory
boxes will result in the form not meeting
validation requirements and you'll have to
start from the beginning. So please
make sure the form is filled in properly.
This form should ONLY be sent by
the legally responsible party who will be in
charge of the Care & Share Program. Granny will receive this
information via email. She will then confirm the
information by corresponding with the person
whose email addressed is used in the form
below. You'll always
have a direct line of communication with
Granny or one of her associates. If one of
Granny's Program Coordinator's has
introduced you to this program please enter
their name and phone number as indicated at
the bottom of this form so that proper
credit is given to that person.
